The collection page always asks for a name. Email is collected by default, and you can turn it off or make it optional. You can add up to 5 custom fields.
Custom fields stay in your dashboard. They do not appear on the Wall of Love unless you use a field as the title on a text testimonial.
Set extra fields
Open the Space and click Edit space in the top right.
In the space editor, scroll to Collect extra information.
Toggle the fields you want, or add custom fields (up to 5). Mark each field required or optional.
Click Update.
You can also fill extra fields on testimonials you already have by editing them.
Questions
Can I skip email?
Yes. Turn off the email field if you do not want to collect it. Uncheck required if email should be optional.
What about testimonials I already collected?
New fields apply only to new submissions. Add the extra details on existing testimonials by editing them.
Can extra fields show on the Wall of Love?
You can display a collected field as the title on a text testimonial. See Edit text testimonials.
